Maa Tara originated with the Samudra Manthan (churning of the ocean). Lord Shiva became unconscious after drinking poison that came out of samudra manthan. Goddess Tara appeared in the form of mother and fed milk. due to which Lord Shiva became concious. After this instance, Lord Shiva and Goddess Tara were known as 'Neelakantha' and 'Neela Saraswati' respectively.
